Mucormycosis (Black fungus) is a designated as rare, rapidly progressive fatal disease of immunocompromised caused by saprophytic fungus family mucorales. Early diagnosis with prompt medical and surgical treatment the only tool available. Rhino-orbito-cerebral most common subtype. In India we saw sudden rise in mucormycosis cases during second wave COVID 19. This necessitated systematic review epidemic A Retrospective multi-centric study was conducted comprising 51 present or recent COVID19 Western Uttar Pradesh positive status presenting to us 14 April 2021- 31 May 2021. Either Type2 Diabetes Mellitus history use steroids high doses all patients. Contribution virulence Delta strain B1.617.2 significant. FESS sino-nasal debridement contributes significantly towards mortality reduction cost total reducing days Liposomal Amphotericin B therapy. management along blood sugar control avoiding dose remain key morbidity reduction.

A case of rhino-orbito-cerebral mucormycosis is presented showing its aggressive nature and progression of disease. The typical clinical features, neuroimaging and histological findings are highlighted in this report. Amphotericin B and surgical debridement remain the mainstay of treatment.
